Notification of Copyright Infringement

To file a claim of copyright infringement, you must send a written communication that includes substantially the following: (these requirements are from Section 512(c)(3) of the Digital Millennium Copyright Act (DMCA)

The physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works are covered by a single notification, a representative list of such works.
Identification of the infringing material. Note: If you follow a specific path to this form, the infringing material should already be pre-populated into this form. (Click Here to see how).
The address, telephone number or email address of the person filing the complaint.
A statement indicating a good faith belief that use of the material is not authorized by the copyright owner.
A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
